We believe it is vital to create an environment of comfort and peace as patients and their families deal with the realities that come with terminal illness.
What is IE Community Hospice?
IE Community Hospice provides a program designed to meet the special needs of patients and their loved ones in time of crisis due to terminal illness for which they do not elect aggressive treatment. Our goal is to assist, educate, and empower the patients and their families about the care and help they need.
Who are the members of the Hospice Team and what are the services provided?
IE Community Hospice provides services covered by this team of skilled disciplines:Medical Doctor, Skilled Nursing (RN& LVN), Hospice Aides, Clinical Social Worker, Spiritual Counselor / Chaplain, VolunteerAdditional Services:
Medication Services, Equipment Services, Therapy Services, Dietary Counseling
What is the program and the benefits provided?
Based on patient’s acuity and level of need, IE Community Hospice provides Routine Home Care, Continuous Care, General Inpatient Care, Inpatient Respite Care, wherever the patient resides may it be Home, Skilled Nursing Facility, Residential Facilities or Assisted Living Centers like Board and Care.
Who pays for hospice care?
Medicare, Private Insurance / HMO’s, and Private Pay
Who are eligible for hospice care?
Patients who have life-threatening illness and are seeking medical, spiritual, personal, and psychological support.
Patients who are not anymore seeking aggressive treatments but now focused on pain management, palliation, emotional as well as spiritual support.
Hospice Levels of Care
1. ROUTINE HOME CARE - Routine visits provided at the place of residence that include intermittent, PRN, and supervisory visits.2. CONTINUOUS CARE - Shifts provided by hospice staff to assist patient through a difficult time if family chooses for patient to remain at home rather than transfer to a hospital.3. INPATIENT CARE - Arrangement with facilities (e.g. hospital) for patient to receive appropriate care beyond what can be provided and managed at home.4. INPATIENT RESPITE CARE - Limited to five (5) days to provide respite to family members or primary caregiver by admitting patient to an institutional setting like skilled nursing facility.
